Microsoft confirmed that after installing the June 9, 2026, security update (KB5094126), users on all supported versions of Windows 11 and Windows 10 may see an odd behavior in the Recycle Bin. When permanently deleting a single item from the Recycle Bin, the confirmation dialog displays an internal system filename such as $Rxxxxx.ext instead of the original filename. The issue also spans Windows Server releases and was confirmed on the Windows Release Health Dashboard.

KB5094126 causes Recycle Bin confirmation glitch

At first glance, this is a cosmetic issue. The file still appears correctly in the Recycle Bin, and restoring it restores the original name without corruption or data loss. The problem is confined to the delete confirmation dialog, where Windows 11 shows the internal Recycle Bin filename ($Rxxxxx.ext) instead of the original name.

The Recycle Bin list itself remains accurate, and restore behavior is unaffected. The impact is limited to the presentation of the final deletion step, which relies on metadata that is now surfaced incorrectly following the June 9, 2026, update (KB5094126).

In practice, this creates a brief inconsistency in the workflow. Users see one name in the list and another in the confirmation prompt, which can slow down review during cleanup tasks, especially in managed or enterprise environments where file verification is routine.

Microsoft confirms Windows 11 Recycle Bin bug

The problem appears to sit in the confirmation dialog’s metadata call. Instead of pulling the user-facing filename from the Recycle Bin index, the operating system exposes the internal staging name used for deletion tracking ($R-prefixed entries). That mapping is normally hidden from the interface layer.

Everything else in the pipeline remains intact. The file system entry is not altered incorrectly, and restore operations still reference the original filename database entry.
